ABSTRACT

Objective:To survey the teaching ability of general practitioners(GPs) in community health centers of Shanghai, and its influencing factors.Methods:A questionnaire survey on the teaching ability of general practice trainers was conducted from January to February 2017. The participants of the survey were GPs with teaching experiences from community health centers in Shanghai selected by a stratified and proportional cluster sampling method. The questionnaire included two aspects: the general information and teaching ability of the participants. The influencing factors of teaching ability were analyzed by univariate and multivariate regression analysis.Results:A total of 1 138 GPs from 116 selected community health centers participated in the questionnaire survey, among whom 63.6% (724/1 138) worked in community teaching bases and 78.5% (893/1 138) had received training for trainers. The total score of teaching ability was 38.00 (30.00, 40.00). The score of the item"interesting in teaching"was (3.46±0.80), 75%—80% of the other questions answered "yes". The score of"application of multi-teaching methods"was (3.78±0.72). Multivariate analyses showed that working for 5—9 and 10—19 years [ OR=7.14 (2.47—11.81), P=0.003; OR=5.32 (1.30—9.33), P=0.009], working in community teaching bases [ OR=13.23 (9.88—16.57), P<0.001] and receiving training for trainers [ OR=17.76 (13.80—21.71), P<0.001] were influencing factors of teaching ability. Conclusion:The main problems related to teaching ability in community GPs are lack of teaching interest and the poor application of multi-teaching methods, the relevant training is necessary for them in the future.

ABSTRACT

Objective To analysis the implementation of the objective structured clinical examination (OSCE) for completion of general practice residency training in Xinjiang Uygur autonomous region.Methods Ninety eight general practice residents took training-completion examination applying OSCE in Xinjiang Uygur Autonomous Region.The examiners (including SP examiners) and examinees were interviewed with questionnaire after the OSCE.Results All 60 examiners and 98 examinees were generally in favor of using OSCE in general practice residency training completion examination;100.0% (60/60) examiners and 86.7% (85/98) examinees thought that it was necessary;98.3% (59/60) examiners and 89.8% (88/98) examinees thought that the OSCE was able to evaluate the clinical competence of general practice;95.0% (57/60) examiners and 78.6% (77/98) examinees thought the OSCE examination stations reasonable;40.8 % (40/98) examinees thought that the difficulty degree of the exam was moderate,35.7% (35/98) and 20.4% (20/98) examinees thought that it was difficult or very difficult,particularly for history writing and case analysis.Conclusions The examiners and examinees recognize the advantages of OSCE in assessment of clinical competence.This study provides useful information for application of OSCE in general practice residency training completion assessment.

ABSTRACT

Objective To evaluate the effect of short-term training of objective structured clinical examination (OSCE) examiner in general practice residency training final examination.Methods Sixty examiners from Xinjiang Uygur Autonomous Region participated in half-day workshop of OSCE.The participants were interviewed with questionnaire before and after the training program and the effect of training was evaluated.Results The study showed that 78.3% (47/60) examiners knew OSCE and understood the concept and implementation steps before the training;21.7% (13/60)had used it before;95.0% (57/60) examiners intended to learn the performance of OSCE.After training most participants completely or basically understood the 6 test stands and implementation methods of OSCE;the participants were highly satisfied with the training duration,contents and methods,and the satisfaction rates were all over 70%.Conclusion The examiners recognize the advantages of OSCE in assessment of clinical skills and have strong intention of application.Short-term training has a certain effect and feasibility.

ABSTRACT

Objective To evaluate the clinical capability of young general practitioners (GPs) in communities in Shanghai.Methods A cross-sectional survey on clinical ability of young GPs was adopted from October to December 2014.Total 211 GPs aged (31 ± 2) years (25-35) from 17 districts of Shanghai participated in the study.Written examination and objective structured clinical examination (OSCE) was applied in the evaluation.Results All 211 GPs (63 male and 148 females) completed the assessment.The overall score of the assessment was (602 ± 83) and the pass rate was 62.6% (132/211).In written examination the average score was (64 ±7) and the pass rate was 72.1% (152/211).In OSCE,the highest score (80 ± 15) and pass rate (92.9%,196/211) was in CPR skill,followed by communication skill [average score:(76 ± 15) and pass rate:85.8% (181/211)].The lowest average score was physical examination (47 ± 15) with a pass rate of 26.1% (55/211),followed by electrocardiogram reading [average score:(84 ±31) and pass rate:39.8% (84/211)].In basic operation station,the lowest score was using funduscope and gynecologial examination (29.8% and 45.4%,respectively).Conclusion Young GPs in Shanghai communities basically master clinical skills,but also have some deficiencies,the training in certain skills need to be strengthened.

ABSTRACT

Objective To assess community health service ability of general practitioners (GPs) with the Standardized General Practitioner Training(SGPT) in Shanghai.Methods One hundred and four GPs receiving SGPT and matched 102 GPs without SGPT were enrolled in this retrospective cohort study from September to November in 2014.Community health service ability,including abilities in basic medical service and public health service were assessed and compared between two groups.Written examination was applied in the public health services ability assessment.Objective structured clinical examination (OSCE) was applied for evaluation of basic medical service abilities.Results The scores of written examination in GPs with SGPT were higher than those without SGPT[(66 ±4) vs.(62 ±7),P <0.01].The OSCE scores in GP with SGPT were higher than those without SGPT [(494 ± 78) vs.(448 ± 80),P < 0.01].GPs with SGPT also abtained the higher scores in consultation competence and clinical skills assessment [(245 ± 44) vs.(225 ±40),P<0.01;(252 ±59) vs.(225 ±61),P <0.01].In OSCE,GPs with SGPT got higher score than those without SGPT in 5 stations (medical history collection,doctor-patient communication,basic operation,ECG interpretation,and CPR) (all P < 0.01);however,no significant differences were observed in 3 stations (physical examination,case analysis and X-ray interpretation) between two groups (all P > 0.05).Conclusions GPs with SGPT have better basic medical service and public health service ability.But some aspects of basic medical service ability need to be strengthened in the training and practice.

ABSTRACT

Objective To survey the current working status of general practitioners (GPs) who received residency training in Shanghai.Methods The current status of general practitioners,who completed residency training in Shanghai from 2013 to 2014,was surveyed with self-designed questionnaire by e-mail.The survey items included current employers,job specification,clinical practice,teaching,scientific research and so on.Results Three hundred and thirteen out of 377 GPs,who completed residency training from 2007 to 2012,participated in the survey (83.0%),including 85 males and 228 females with a mean age of (31.2 ± 2.6) years.Among all participants,298 (95.2%) worked in community health service centers,12.8% (40/313) of them were involved in general practice teaching,9.9% (31/313) undertook management,9.3% (29/313) did academic research in community,and 7.7% (24/313) had chance for overseas study.The survey also showed that 134 person/times received various commendations and awards.Conclusion Most GPs who completed residency training are undertaking clinical practice in community health service centers,and they are also well involved in management,teaching,and research.

ABSTRACT

Objective To study the application of accreditation standards for trainers of general practitioners (GPs) in community health centers.Methods The questionnaire was designed according to the accreditation standards for GP trainers in community health centers,which involved professional quality,clinical competence in primary care and teaching abilities.According to typical sampling,583 trainers from 56 community teaching bases in Shanghai were selected for the survey.Results Among 583 participants,the effective respondents were 505 with a response rate of 86.6%.The professional quality was highly recognized by the participants,and more than 80% of trainers met the requirement.For the ability of primary care,81.4% (411/505),79.8 % (403/505) and 69.5 % (351/505) of participants were well qualified with the abilities of management for hypertension,type 2 diabetes and coronary heart disease,respectively,while only about 40% had abilities of management for epilepsy,hypothyroidism and chronic mental diseases.75.4% (381/505) participants fully mastered the principle of first aid skills and CPR,while only 24.8% (125/505) had abilities of management for conjunctival foreign body and nasal foreign body,34.5% (174/505) had the abilities of debridement and gastrolavage.For the index of teaching abilities,most trainers had insufficient teaching capacity and experience in the community.Only 48.5% (245/505) and 40.2% (203/505) of trainers were familiar with the teaching methods of cased-based learning and problem-based learning,respectively.Significant difference was found in the scores of teaching abilities among trainers from community health centers in different locations (P < 0.05),such as using appropriate teaching methods (F =3.45,P =0.033),monthly assessment and feedback to students (F =5.14,P =0.006) and ensure at least 4 hours of teaching time every week(F =4.96,P =0.007).No significant differences were found in the scores of professional quality and clinical competences in primary care (P > 0.5).Conclusion The trainers of general practitioners in community health centers meet the requirement of accreditation standards in the aspects of professional quality and clinical competences in primary care,while the teaching abilities remained to be improved and should be strengthened in the future.

ABSTRACT

Objective To evaluate the simulation of standardized patients (SP) in objective structured clinical examination (OSCE) for general practice residency training.Methods The simulation levels of 15 SPs were evaluated by 37 examiners with a rating scale of OSCE Examiner Feedback on SP performance from Melbourne University School of Medicine.The evaluation was conducted in history-taking and doctor-patient communication stations of OSCE for general practice residency training.Results The overall scores of SP performance were high and most examiners considered that the performance of SP was realistic and consistent.Among 9 items of rating scale,more than 90% of the examiners considered that SP performance met or completely met the requirements of OSCE,except the item of preparation for clothing and making up before role playing.Six items of performance in history-taking station and 4 items in doctorpatient communication station reached 100% satisfaction from the examiners.T test was used to compare the difference of SP performance between the morning and afternoon,and there was no significant difference (P > 0.05).Conclusion The study showed for this group of SPs,the realistic degree and consistency of performance meet the requirements of OSCE,and its stability is not affected by long time continuous performance.

ABSTRACT

Objective To develop an assessment criteria for general practitioner (GP) trainers in clinical training base.Methods Based on the extensive literature review and expert interview,a preliminary index system was established with 4 first-grade indicators and 45 second-grade indicators for GP trainers in clinical training base.During 2012 to 2013,a two-round Delphi consultation was conducted to establish the assessment criteria.Results Thirty seven experts engaged in clinical practice,training and managers of GP training program from clinical training bases,medical schools,or health bureau in Shanghai were invited for Delphi consultation.Among them 64.8 (24/37) % were engaged in GPs training over 5 years,78.4% (29/37) worked as GP trainers,70.2% (26/37) with high-level professional title.In two round consultations,the activity coefficient of the experts were 97.3% and 100.0%,authority coefficient were 0.817 and 0.855,and harmonious coefficient of importance of the evaluation were 0.188 (P < 0.01) and 0.278 (P < 0.01),respectively.The opinions among the experts were consistent from the first round to the second round.The assessment criteria for GP trainers in clinical training bases was established with 4 firstgrade indicators and 27 second-grade indicators.The 4 first-grade indicators were professionalism,clinical practice components,teaching quantity & components,basic conception of general practice,with weight coefficient of 0.253,0.253,0.251 and 0.243,respectively.Conclusion The established assessment criteria for GP trainers is credible and comprehensive,which can be applied in evaluating the comprehensive capacity of trainers in clinical training bases.

ABSTRACT

To investigate the status of application of Chinese Hypertension Prevention and Treatment Guideline (Guideline) by general practitioners (GPs) in Shanghai.A questionnaire survey was conducted among 260 GPs from 8 communities in Xuhui District of Shanghai during March 2011 to December 2012.Among all participants,94.2% knew the Guideline.The overall correct rate was 56.2%; that of epidemiology,diagnosis,treatment,two-way referral and community management of hypertension was 49.2%,54.5%,60.6%,56.9% and 32.3%,respectively.There were significant differences in correct rates of the questionnaire between GPs with different age,gender,education and community (P =0.000,0.035,0.000,0.000).The correct rates of epidemiology and treatment in GP teaching bases (58.5% and 62.6%) were higher than those in non-teaching bases (39.2% and 58.4%) (P =0.002,0.040).But there was no difference in overall correct rate between these two groups (P =0.095).The results indicate that the understanding and application of Hypertension Guideline by General practitioners in community is insufficient.Teaching hospitals should carry out systematic,comprehensive,sustained,targeted training course in order to improve the diagnosis and treatment levels of hypertension for general practitioners in community.

ABSTRACT

Objective To survey the double-preceptor system in standardized residency training of general practitioners (GPs).Methods A questionnaire survey was conducted among 48 residents,53 clinical and community preceptors during May and June 2013.Results The preceptors mostly agree with the necessity to carry out double-preceptor system.82.8% (24/29) clinical preceptors,95.8% (23/24) community preceptors and 89.6% (43/48) residents considered that it was very effective.Less than half of the clinical(13/29) and community (11/24) preceptors knew very clearly about their responsibilities and obligations ; 93.1% (27/29) clinical preceptors and 95.8 % (23/24) community preceptors hoped to receive corresponding training courses.72.4% (21/29) clinical preceptors and 75.0% (18/24) community preceptors communicated with residents frequently,75.9% (22/29) clinical preceptors thought residents were not actively involved was the main obstacle in the implementation of double-preceptor system.The guide contents were focused on clinical skills,scientific research and how to pass assessments.The main approaches were interviews,checking the quality of medical instruments and small lecture.Only 6.9% (2/29) clinical preceptors conducted outpatient teaching,83.3% (40/48)residents thought tutors did not knew clearly about their responsibilities and obligations was the main obstacle in the practice of doublepreceptor system.Regarding how to evaluate the double-preceptor system,clinical and community preceptors tended to use examination results and feedback of residents.Clinical preceptors,community preceptors and residents all gave low comments about the preceptor-resident contact handbook.87.5% (21/24) community preceptors thought lack of corresponding incentive mechanisms was the main obstacle in the practice of double-preceptor system.Conclusions Double-preceptor system has been widely recognized by both preceptors and students.In order to improve the system,we should conduct tutor training courses regularly and establish an effective mechanism to assess and evaluate tutors.

ABSTRACT

Objective To establish a set of accreditation standards for trainers of general practitioner (GP) in community health service centers.Methods A modified Delphi expert consultation was conducted during December 2012 and September 2013.Thirty two experts of general practice from different teaching hospitals and community health service centers were invited for three rounds Delphi questionnaires.A set of accreditation standards for GP trainers in primary care was established through data analysis,synthesis and process.Results Expert activity coefficients were 100.0% for three rounds consultation,the authority coefficients were 0.894,0.882 and 0.893,respectively.The opinion coordination coefficients of experts were 0.156,0.166 and 0.215,respectively (P =0.000).The developed accreditation standards system comprised of three first-grade indicators and 14 second-grade indicators.The weight coefficients of three first-grade indicators,namely professional profiles,clinical competences in primary care and teaching abilities,were 0.339 8,0.335 6 and 0.324 6,respectively.Conclusions The established accreditation standard system is credible,which would appropriately guide the selection of GP trainers in primary care from three dimensions and enhance the quality of standardized training of general practitioners.

ABSTRACT

Objective To develop criteria for trainers of general practitioners in clinical training bases.Methods A primary version of criteria was established through literature review and expert interview.Thirty-two experts with middle-level professional title or above,who had 10 years of working experience or more in general practice,teaching and administration,were invited for 3 rounds of Delphi consultation during March and October 2013.Results The criteria consisted of 3 first-grade indicators and 12 second-grade indicators were established.The first-grade indicators included professional quality,clinical competence and teaching capability.The weight coefficient of first-grade indicators were 0.332 0,0.336 0 and 0.332 0,respectively.For 3 rounds of consultation the activity coefficient of experts was all 100% ; the authority coefficients of experts were 0.881,0.897 and 0.883,respectively; and the harmonious coefficient of importance of the evaluation were 0.136,0.127 and 0.204,respectively (P < 0.01).Conclusion The established criteria are credible and important for the selection of trainers of general practitioners in clinical bases,which would improve the quality of standardized training for general practitioners.

ABSTRACT

Objective To assess the continuing medical education (CME) and training needs for gereral practitioners (GPs) in urban and suburb of Shanghai.Methods Gereral practitioners in urban (n =710) and suburb (n =843) of Shanghai attending two municipal training programs were interviewed with self-designed questionnaire during July 2011 to May 2012.Results Urban group included 710 GPs whose mean age were (36.7 ± 6.8) y and 33.0% were male.Suburban group had 843 GPs whose mean age were (36.6 ± 5.1) y and 33.2% were male.The survey showed that 84.6% of urban group and 85.6% of suburban group had attended CME before.There was no statistical difference in the overall satisfaction of CME between two groups (Z =-0.895,P =0.371).The propotion of satisfied and very satisfied was 45.1% and 13.8% in urban groupt,while it was 49.5% and 9.1% in suburb group.The overall satisfaction rates of enhancing basic theories (Z =-2.296,P =0.022) and skills (Z =-2.013,P =0.044) and CME textbooks (Z =-2.819,P =0.005) were higher in suburban group than those in urban group.Both of them agreed that the contents of CME were divorced from reality of practice (60.3% vs.65.2%,x2 =3.213,P =0.082).The percentages of complain of insufficient training time (19.7% vs.11.0%,x2 =18.045,P < 0.05) and contents (7.5% vs.4.3%,x2 =5.747,P =0.019),and weak leadership commitment (14.3% vs.6.5%,x2 =19.95,P < 0.05) were higher in suburban group than those in urban group.On the contrary,the delivery of repeated contents (24.8% vs.34.8%,x2 =15.327,P <0.05) and long training time (16.4% vs.28.2%,x2 =26.055,P < 0.05) were lower in suburban group than those in urban group.Using case study in the teaching was the common choice of both groups (73.3% vs.78.0%,x2 =0.635,P =0.426).Compared to urban group,suburban group was more likely to choose lecture of theory (63.6% vs.50.8%,x2 =6.712,P =0.010) rather than problem based learning (43.1% vs.51.9%,x2 =4.273,P=0.010).Conclusions Both urban and suburban GPs in Shanghai are generally satisfied with the CME programs.There are some discrepancies in training needs of CME between urban and suburben area in Shanghai,indicating that the CMB programs need to be designed according to the needs of trainees in urban or suburb areas.

ABSTRACT

Objective To establish a assessment system for comprehensive clinical competency of general practice residents.Methods An assessment system for comprehensive clinical competency of general practice residents was developed with modified Delphi expert consultation during November 2010 to October 2011.Thirty three experts in clinical practice,education and administration of general practice were invited for two-round Delphi questionnaire consultation.Results In two-round consultation,the expert activity coefficient was 100%,the authority coefficient was 0.852 and 0.855,and the coordination coefficient for the importance of the indicators was 0.228 and 0.245 respectively (P =0.000).The opinions among the experts became consistent from round 1 to round 2.An assessment system was established,which consisted of 5 first-level (professionalism,communicate skills,clinical competency and professional development ability) and 36 second-level indicators.Conclusion The index system established in this study may be used as references for evaluating comprehensive clinical abilities of general practitioners systematically and objectively.

ABSTRACT

Objective To survey on the knowledge,attitude and practice about hypertention in elderly patients and its relation to blood pressure control.Methods A sampling survey on the knowledge,atitude and practice about hypertention were carried out in a target population of 47 572 elderly hypertensive patients from 5 communities in Xuhui District of Shanghai during May 2011 to July 2011.Total 3800 questionairres were distributed and 3328 responded with a response rate of 87.5%.Results Among 3328 responders,3036 were aged above 65 years with a mean age of (76 ±7) years and 43.3% were males.The rates of knowledge about hypertension were 88.5%-97.8%.General practitioners in out-patient service were the major source of knowledge.Raults revealed that 81.9% had received health education before;81.8% requied a regular health education; 67.2% had self-tested blood pressure weekly; 89.6% controlled salt intake; 66.8% attended outdoor activities weekly with (5.6 ±2.1) times a week and (45 ±28) min each time.The blood pressure control rate in uncomplicated hypertensive patients was higher than those complicated with diabetes,coronary heart disease or stroke (62.3 % vs.17.0%,P < 0.01).Knowing high blood pressure was related to salt intake (OR =3.364,P < 0.01),hoping a regular health education about hypertension (OR =1.718,P < 0.01) were factors contributing to blood pressure control in uncomplicated hypertensive patients; while self-testing blood pressure once or twice a month(OR =2.572,P < 0.01)was contributing factor to blood pressure control in complicated hypertensive patients.Conclusions Elderly hypertensive patients from communities in Shanghai have enough knowledge,positive attitude but lack practices.General practitioners should carry out personalized and stronger health education focued on behavioral changes for those patients.

ABSTRACT

Objective To investigate the training needs of general practioners (GPs) in Shanghai and to analyze the related factors.Methods A questionnaire survery on the training needs was conducted among 428 GPs who attended a municipal training program of clinical compentance in Shanghai during July 2011 to February 2012.Results Among 428 GPs receiving the survey,23.9% (102/427) expressed great willingness for training; 40.2% ( 169/421 ) preferred part-time and 41.3% ( 174/421 ) preferred on-job training; 31.7% (377/1188) wished to have case analysis method of teaching.In the aspect of training contents,common clinical skills were ranked the first,accounting for 32.1% (1376/4280) of the choices,on the other hand,health care for women,children and the elderly in community was ranked the last,accounting for 18.9% (485/2568) of the choices.Logistic regression showed that the gender (OR =2.194,P=0.010),age (OR=0.435,P=0.011) and education levels of GPs (OR=0.293,P=0.006) were related factors of training needs.Conclusion The survay reveals that training needs of GPs are strong in Shanghai and GPs prefer short-term training in real clinical situations.

ABSTRACT

Objective To assess consultation competence of resident general practitioners in Shanghai. Methods Standardized patients (SPs) were employed as an assessment tool in exploring consultation competence of resident general practitioners ( GPs) , including clinical and communication skills. Clinical skills included information collection, clinical thinking, health education, out-patient history taking and recording and reception time management, and patient-physician communication skills included building a good relationship, information collection and feedback, understanding issues from patient's perspective, sharing information and obtaining informed consent and close of consultation. Score rate defined as actual score of each item divided by total score of the domain was employed to assess competence of GPs in varied aspects. Results There were 157 resident GPs, 52 men and 105 women, with an average age of (28 ±2) years. Overall score rate of clinical skills averaged (76 ±10) percent, and score rate of each item ranking from the highest to the lowest was reception time management (91 ±4) percent, out-patient history taking and recording (80 ±14) percent, information collection (76 ±12) percent, clinical thinking (74 ±17) percent, health education (68 ±1) percent. Overall score rate of communication skills averaged (74 ±12) percent, and score rate of each item ranking from the highest to the lowest was building a good patient-physician relationship (86 ±3) percent, information collection and feedback (80 ±5) percent, sharing information and obtaining informed consent (74 ±15 ) percent, close of consultation (62 ±27 ) percent and understanding issues from patient's perspective (43 ±0) percent. Conclusions Resident GPs have improved their consultation competence to certain extent after standardized training, but there still exist some weakness to be improved in them.

ABSTRACT

General practice (GP) training program in Shanghai has been carried out in Zhongshan Hospital of Fudan University,which consists of theoretical study,clinical rotation and community practice.The contents of assessment include clinical competences,theoretical knowledge,and research capability; the methods of assessment include objective structured clinical examination,multiple choice tests,and assay writing.During the program implementation Zhongshan Hospital exert great efforts to set up the teaching management framework,quality control system,and to improve teaching methods in order to upgrade the clinical capability training for the residents.
